This policy covers this website: what it collects when you contact us through it, and what happens to that information. It is not our Notice of Privacy Practices, which covers your medical records and is a separate document.
What the appointment form collects
When you send an appointment request, the form asks for your name and a phone number, and optionally an email address, a preferred center, a preferred time of day and a short note. Nothing else is collected, and nothing is required beyond a name and a number to reach you on.
The request is emailed to the practice so that somebody can call you back, and a copy is kept in this website's own admin area so that a request cannot be lost if an email goes astray. Only staff who can sign in to the website can see it. It is not added to your medical record by the website, and the website is not a patient records system.
Why we ask you not to put medical details in the form
The form is a normal web form and the message it generates is ordinary email. That is fine for "please call me back", and it is not the right place for your diagnosis, your test results or anything else you would consider private. We will take your history properly when we speak to you, on the phone or in person.
If you do include medical details, we will handle them carefully, but we would rather you did not have to rely on that.
Cookies and tracking
This site does not set advertising cookies and does not track you across other websites. Pages that show a map embed content from Google, and Google may set its own cookies when that map loads. If you would rather it did not, use the address and the phone number on the page instead of the map.
Talking to us instead
You never have to use the form. Call either center and we will take the request over the phone: Arlington 817-417-1132, Fort Worth 817-438-0259.
