You do not need to print or complete anything before your first visit. Our registration forms are completed at the front desk when you arrive, and the team will walk you through them. What helps most is bringing the right documents with you.
What to bring to your first visit
Gather as much of this as you can. If you do not have all of it, come anyway. We would rather see you and chase a report afterwards than have you put off the appointment.
- Photo ID
- Your insurance card
- A current list of your medications, including anything over the counter, with doses
- Pathology reports relevant to your diagnosis
- Recent imaging reports, and the imaging discs themselves if you have them
- Records from other physicians involved in your cancer care, including the name and number of the doctor who referred you
- A written list of your questions
Completing forms at the front desk
Registration and consent forms are completed in person before your first consultation. Allow a little extra time on your first visit for this. If reading or writing is difficult for you, or you would like someone to go through the forms with you, tell the front desk and they will help.
Bringing someone with you is a good idea for the whole first visit, not only for the paperwork. Two people remember more than one. Read what to expect for how the visit runs from there.
